Wow, is that a procedure they cover? You'll likely have to lobby to the insurance company to change what they cover. Good luck on that!

What service is the therapist providing? Pulmonary therapy? Not that is matters... If an insurance doesn't cover smoking cessation as a diagnosis, the service won't be covered. period. If the patient needs treatment from a respiratory therapist to quit smoking, maybe there's an under or overlying medical condition that should be represented with a better/different diagnosis. What CPTs are you using? and what icd9s? How does this service pertain to 'medically necessary' treatment?
