They don't cancel each other out, and her lungs would probably be the most affected. Exercise would definitely help but her organs would be much better off if she quit smoking.

Actually, strenuous exercise will put a person more at risk for immediate damage, because when the pulse rate is raised, the person is effectively pumping the deadly toxins (associated with cigarettes and other tobacco products) more rapidly through their system.
The exercise will not cancel out the effects of nicotine.

First of all, I do not know what an 'athlet' is, but I assume you mean athlete.
Why haven't you looked at this situation and used the indestructible properties to analyze it?
