It can. My grandmother had to get open-heart-surgery a few years back because of it. Sadly though, right out of surgery, she was asking for her cigarettes. She is working on quitting now, almost 7 years later, but hey, better late than never, right?
Anyways, it can. She had to get open-heart-surgery, and a few stints put in after that, and the doctors have all said it's got to do with her smoking.

Smoking causes vascular disease which means that it damages the blood vessels including the coronary arteries (blood vessels that supply the heart with blood).

Smoking doesn't cause heart disease directly - it depletes certain vitamins and minerals that help maintain proper health. The tars and nicotine are also toxic and enter the bloodstream through the lungs, causing dehydration.

With dehydration, the blood thickens and becomes acidic. Coupled with the toxins from the tar and nicotine, these cause tiny cuts and abrasions to the inside of the artery walls as they are pumped through. This damage could peel away and become an embolism in the brain or other organ if it wasn't for cholesterol. Cholesterol is a good thing - as long as it's caught early before it accumulates and does any damage. The cholesterol is produced in response to the damaged arteries - it acts like a patch or a band-aid until the body can make the needed repairs.

Absolutely it does. It is a major risk factor for heart disease and all other vascular diseases such as strokes, peripheral vascular disease, and abdominal aortic aneurysms. It also increases the risk of many cancers outside of the lung. Smoking 20 cigarettes daily increases the risk of heart attack 6 times in women and 3 times in men. Smoking has also been shown to increase death from heart disease significantly. There are a lot of statistics on smoking and heart disease, and I would be happy to share them with you if you would like.
